Affordable Care Act Applications Issued in Short Form

The Centers for Medicare & Medicaid Services (CMS) has issued a Short Form application for individuals applying for health insurance under the Affordable Care Act, often referred to as Obamacare.  The short form is an improvement over previously released draft applications as it reduces the application to 3 pages, down from 21 previously issued.  Required information on the short form includes name, address, Social Security number, citizenship details, job status and certain current health coverage information.  The short form application becomes effective on October 1, 2013, the date the state-based health insurance exchanges are scheduled to begin open enrollment.  Click the link below to obtain the new 3 page Application for Health Coverage & Help Paying Costs (Short Form).
